This fixes the scenario where a note file referenced by a todo, event,
or appointment is modified from outside the program in such a way that
it cannot be opened by the program, resulting in a segmentation fault.
The least surprising way to proceed is to ignore the note.

Show default description "(empty description)" for all types of items

terminal in any way. Create a new session for them.

When a scrollbar is on display in APP or TOD windows, the right
vertical border (outside the scrollbar) is not highlighted
when the window is selected.
The scrollbar is always highlighted:
- when APP or TOD is deselected
- in configuration windows where borders otherwise are not
The patch moves the scrollbar parameters (except highlight)
from arguments of draw_scrollbar() to the function itself.
The highlight argument was 1; instead it is set higher in
the call hierarchy (wins_update_panels()) and passed on down.

To prevent from illegal memory access, we checked whether the list of
todo items is empty before performing an interactive todo item action
such as flagging or removal. However, since 1a4bf2b (Add a "hide
completed" view to the todo panel, 2016-01-17), it is possible to hide
completed items from the todo panel. Thus, it may occur that the todo
list box is empty while the list of todo items is not.
To detect such situations as well, teach todo_get_item() to return NULL
if the requested item does not exist. Then, instead of checking whether
the todo item list is non-empty, make sure that the item returned by
todo_get_item() is non-NULL when performing an action.

In the todo panel, all uncompleted items are currently prefixed with
their priorities. Drop this prefix from items with priority 0, i.e.
undefined priority.

This adds support for rows that cannot be selected. Such rows can be
used for section headings and the like.

Make use of the generic scroll window implementation for the calendar
view. Note that this is useful despite the panel not needing a scroll
bar, since the scroll window functions can be used to draw the panel
border and take care of relative positions.

This completes our switch to the Linux kernel coding style. Note that we
still use deeply nested constructs at some places which need to be fixed
up later.
Converted using the `Lindent` script from the Linux kernel code base,
along with some manual fixes.
